ASSUMPTION OF RISK, WAIVER, RELEASE OF LIABILITY, INDEMNIFICATION, & COVENANT NOT TO SUE
IN CONSIDERATION OF my child being allowed to participate in any way in Bandits Lacrosse Club, Inc. related events and activities (including practices, games, tournaments, travel, and use of equipment/facilities), the undersigned acknowledges, appreciates, and agrees that:

Risks Involved: Lacrosse is a high-contact sport with significant risks of injury and illness (including communicable diseases such as COVID-19), from impacts with sticks, balls, other players, goalposts, field conditions, or faulty equipment. These risks include sprains, fractures, concussions, permanent disability, and death. While rules, protective gear (e.g., helmets, gloves), and coaching may reduce risks, they cannot eliminate them entirely.
Assumption of Risk: I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (defined below) or others, and assume full responsibility for my child's participation. I willingly agree to comply with the program's rules, terms, and conditions. If I observe any unusual concern in my child's readiness, equipment, facilities, or the program itself, I will immediately remove my child from participation and notify the nearest official.
Release and Waiver: I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Bandits Lacrosse Club, Inc., its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to my child's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
Indemnification: I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives, and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my child's participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law (including reasonable attorneys' fees).
Medical Authorization: In case of injury or illness, I authorize Releasees to seek, administer, or consent to emergency medical treatment for my child, including transportation to a medical facility. I agree to pay all associated costs and waive any claims related to such treatment.
Parent/Guardian Explanation: I, the parent/guardian, assert that I have explained to my child the risks of lacrosse, his/her responsibilities for adhering to rules and using equipment properly, and that my child understands this agreement.
Child Acknowledgment (for participants 12 years and older): I have read (or had read to me) this agreement, understand the risks, and agree to follow all rules.

I, FOR MYSELF, MY SPOUSE, AND CHILD, HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
PHOTO AND VIDEO RELEASE: I hereby grant permission to Bandits Lacrosse Club, Inc., to take and use photographs and/or digital images of my child for use in news releases, educational materials, promotional materials, and/or other publications (printed, electronic, websites, or social media). My child's name and identity may be revealed in accompanying text. I authorize this use without compensation to me, and all materials shall be the property of Bandits Lacrosse Club, Inc., in perpetuity. I release the Club from any claims arising from such use.
I HEREBY COVENANT AND AGREE NOT TO SUE any of the Releasees based on any claim arising from my child's participation in Bandits Lacrosse Club, Inc. activities.

I have read and agree to all the terms and conditions of the Assumption of Risk, Waiver, Release of Liability, Indemnification, and Covenant Not To Sue?
